Family Medicine Advanced Practice Provider - Community Health SettingLocation: Houston, TXSetting: Outpatient / Primary CareEmployment Type: Full-Time, permanent positionPosition OverviewThis position provides advanced clinical care in a family medicine setting under the supervision and delegated authority of a physician. The provider delivers comprehensive primary care services across the lifespan, including preventive care, chronic disease management, and acute care visits in a community-based outpatient clinic environment.Position Highlights Provide direct patient care in an outpatient primary care setting Diagnose and manage common medical conditions Perform wellness visits and preventive care services Manage chronic health conditions across all life stages Participate in patient education and care planning Collaborate with physicians and care team members Support quality initiatives and clinical best practicesPrimary Responsibilities Perform patient assessments and physical examinations Conduct wellness visits and health risk assessments Develop and implement patient-centered treatment plans Order and interpret diagnostic testing Provide counseling and education related to diagnosis and care Maintain accurate documentation in the electronic health record Identify high-risk patients and coordinate referrals as needed Support continuity and coordination of care Utilize evidence-based practice standards Participate in quality improvement and peer review activities Assist with development and review of clinical protocols Provide indirect supervision of clinical support staff Participate in meetings and team functions Perform additional duties as assignedQualifications Graduate of an accredited Physician Assistant programOR Master of Science in Nursing programCertifications & Licensure Certified by AANP, ANCC, or NCCPA Active and unrestricted license in the State of Texas(Nurse Practitioner or Physician Assistant) Current Basic Life Support certificationSkills & Experience Knowledge of outpatient primary care principles and treatment standards Ability to assess, diagnose, and manage common medical conditions Strong documentation and clinical decision-making skills Ability to work effectively with diverse patient populations Commitment to team-based, patient-centered careIdeal Candidate Demonstrates compassion and patient-focused care Values collaboration and communication Comfortable working in a community health environment Dedicated to quality, safety, and service excellence by Jobble
