APP Internal Medicine opening in Auburn, NYSyracuse 28mWe are seeking a full-time NP/PA to join our team, providing primary care services in a skilled nursing facility located in Auburn, New York. Monday - Friday schedule Providing comprehensive primary care services to a manageable patient panel of 15-18 patients per day Collaborating with interdisciplinary teams to optimize patient outcomes Ensuring smooth transitions of care with hospice, palliative care, and home care agencies Maintaining accurate and timely documentation in our EMR system (Gehrimed)Qualifications Graduate degree as a nurse practitioner or physician assistant Active and unrestricted NP or PA license in New York DEA Strong clinical assessment and diagnostic skills Excellent communication and interpersonal abilities Experience in post-acute care preferred, but not required (training available)Compensation & Benefits Expected compensation is estimated compensation range of $130,760 to $154,911 with no cap on productivity income potential.
