The ideal candidate will be a collaborative professional who can effectively manage the needs of both hospital discharge patients and the wider community. This role offers a unique opportunity to work with an interdisciplinary team and make a direct impact on patient care in a vibrant community. About the Role As a Community Outpatient Pharmacist, you will staff the outpatient pharmacy, collaborating closely with both inpatient and outpatient pharmacy teams and all area healthcare providers. You will be responsible for fulfilling hospital discharge orders while also serving the needs of our community patients. This role also includes rotating Medication Reconciliation Pharmacist shifts with other staff. Schedule The retail pharmacy hours are Monday through Friday from 7:00 am to 5:30 pm and Saturday from 8:00 am to 4:00 pm. The pharmacy is closed on Sundays. This position requires rotating holidays and Saturdays. Qualifications Education: A graduate of an ACPE-accredited School of Pharmacy with a Bachelor of Science or Doctor of Pharmacy (PharmD) degree. License & Certifications: A current license to practice pharmacy in the state of Colorado, or the ability to obtain it within 60 days of hire. You must also be Immunization Certified with an active BLS card. Skills: The ability to work independently with minimal supervision, handle frequent interruptions, and adapt to changes in workload and schedules. You should be able to set priorities, make critical decisions quickly, and respond to emergency requests. This role requires meeting the pharmaceutical needs of a wide range of patients, including neonatal, pediatric, adolescent, adult, geriatric, psychiatric, oncologic, obstetric, chemically dependent, and rehabilitation patients. Experience: Community pharmacist experience is preferred.
