Your Opportunity We are seeking a Physical Therapist who is responsible to provide high-quality physical therapy services to clients ensuring compliance with all professional standards, regulatory requirements, and Solara Care Solutions policies. Key Responsibilities: Responsible for the evaluation and treatment of clients who demonstrate disorders of musculoskeletal deficiencies, neurological conditions, balance disorders, and transfer/gait deficits. Evaluation and treatment will only be conducted after a written order is received from the physician. Responsible for the screening of clients of the community as follows: New Community clients: Within 24 hours of admission Long term clients: Upon receipt of a "significant change" form and/or upon referral from nursing. Responsible for attending all required community meetings and in-services, including Medicare meetings, family meetings, care plans QUALIFICATIONS: Minimum Education Required: Masters of Science Degree or Bachelor of Science Degree in Physical Therapy.
