Admission Coordinator Department: Outpatient Chemical Dependency Location: Evelyn Brandon Health Center Hours Per Week: 40 Schedule: Monday - Friday, 8:30AM - 5:00PM Summary: The admission coordinator must be knowledgeable of the clinical aspects of chemical dependency in order to coordinate admission of patients into the Rochester Regional Health's chemical dependency programs. The employee must participate as a contributing member of an interdisciplinary team in the appropriate admissions onto the program, and provide superior customer service by modeling the Brand Promise and Core Values. The Admission Coordinator is also responsible for reviewing incoming referrals from both internal and external sources for appropriate level of care, work with the finance department around insurance verification, coordinate with program managers of various treatment sites around appropriate referrals and solidify bed dates for patients who meet criteria. Responsibilities: Responsible for reviewing incoming referrals for patients seeking detox, inpatient, outpatient, and residential services. May also assist with other program eligibility assessment and enrollment as needed. Work with internal and external referral sources around patient referrals and admission/non-admission dates and reasons. Knowledge of admission criteria for inpatient level of care including DSMV criteria and Locatdr. Must be able to communicate amongst various disciplines including: medical staff, nursing staff, clinical staff, psychiatric staff, and program managers when necessary. Must be able to work closely with the finance team to work on insurance verifications to reduce denials from insurance companies and maximize revenue. Must be able to cross train across the various inpatient units and provide back up coverage as needed to team members within the Admission Coordinator role. Responsible for daily beginning of day and end of day census. Responsible for keeping a set minimum census at the various locations. Required Qualifications: Associate's Degree or equivalent work experience required. Preferred Qualifications: Bachelors degree preferred. 1-2 years relevant experience in chemical dependency care preferred. OASAS approved Qualified Health Professional (CASAC, LMSW, LMHC, LMHC-P) is preferred but not required Education: Licenses / Certifications: Physical Requirements: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. Pay Range: $19.00 - $25.00 City: Rochester Postal Code: 14608 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
