Medical Coordinator The Medical Coordinator is responsible for arranging and coordinating appropriate health services for children in care, including urgent, emergency, and preventive care. This role ensures children receive timely and comprehensive medical care while maintaining complete and confidential medical records in accordance with agency policy, program requirements, and applicable licensing regulations. The Medical Coordinator ensures effective communication of children's medical needs to case managers, clinicians, and program staff; schedules healthcare appointments; coordinates transportation to medical appointments; ensures qualified interpretation and translation services are available when needed; and serves as a liaison to community healthcare providers. This position supports the organization's commitment to trauma-informed, culturally responsive, and high-quality care for all children served. Essential duties and responsibilities include arranging and coordinating appropriate health services for children in care, maintaining complete, accurate, and confidential medical records, ensuring appropriate communication of medical and health needs, scheduling and coordinating medical, dental, vision, behavioral health, and specialty healthcare appointments, coordinating transportation to medical appointments, ensuring qualified interpretation and translation services are available for medical appointments and healthcare communications when needed, serving as liaison between the program and community healthcare providers, hospitals, specialty clinics, and public health entities, monitoring medical compliance requirements, preparing, compiling, submitting, and maintaining medical documentation, reports, and records required for programming, licensing, and internal compliance monitoring, reviewing programs portals and internal systems to ensure required medical documentation is uploaded accurately and promptly, providing medical status updates to authorized sponsors, family members, and staff as appropriate and in accordance with confidentiality regulations, participating in multidisciplinary team meetings and service planning to ensure medical needs are incorporated into individualized service plans, supporting discharge planning by ensuring continuity of medical care and transfer of appropriate medical documentation, adhering to mandated reporting laws and child abuse reporting requirements, attending required trainings, meetings, and professional development activities, and performing other duties as assigned by supervisor or leadership. Supervisory responsibilities include none. Competencies include problem solving, communication, teamwork, planning and organization, professionalism, and judgment. Preferred bilingual in Spanish but not required. Qualifications include minimum of 1 year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care, and any of the following: licensed practical nurse (LPN/LVN) or higher health-related qualification (e.g., licensed registered nurse (RN) with a bachelor's degree), or bachelor's degree in nursing, public health, health administration, social work, or other relevant health-related field. Must be at least 21 years of age, valid driver license, and ability to pass required background clearances. Licensed practical/vocational nurse (LPN/LVN), registered nurse (RN), or bachelor's degree in a relevant health-related field with at least one year of experience in healthcare coordination, child welfare medical services, or public health programs. Experience working with children, families, or vulnerable populations preferred. Bilingual in Spanish is requested but not required. Read, analyze, and interpret business, professional, technical, or governmental documents. Write reports, business correspondence, and procedure manuals. Effectively present information and respond to questions from managers, customers, and the public. Proficiency in MS Office (Word, Excel, Outlook). Proficient in the use of standard office equipment. Electronic medical record and UC portal software. Work environment characteristics include moderate noise level.
