Benefits: 401(k) Competitive salary Dental insurance Health insurance Opportunity for advancement Paid time off Training & development Vision insurance IDD Medical Coordinator Department: Operations / Clinical Supports Reports To: Clinical Director FLSA Status: • Exempt • Non-Exempt Employment Type: Full-Time Location: Residential & Community Programs Position Summary The IDD Medical Coordinator is responsible for coordinating, tracking, and supporting all medical, psychiatric, and health-related services for individuals with intellectual and developmental disabilities (IDD) served by the agency. This role ensures compliance with ODP 6400/6100 regulations, promotes continuity of care, supports Direct Support Professionals (DSPs), and serves as a liaison between providers, families, supports coordinators, and internal leadership. Essential Duties & Responsibilities Medical Coordination & Oversight:• Coordinate primary care, specialty, dental, vision, psychiatric, and therapy appointments for individuals served • Maintain and track annual physicals, dental exams, vision exams, immunizations, and lab work • Ensure timely follow-up on abnormal results, referrals, and provider recommendations • Support transitions of care (hospitalizations, ER visits, discharges, new admissions) Medication & Health Monitoring:• Coordinate with pharmacies, prescribers, and residential teams regarding medication changes and refills • Ensure medication orders are accurate, current, and aligned with treatment plans • Monitor medication compliance issues and support corrective actions • Track and report medication-related incidents in accordance with policy Documentation & Compliance:• Maintain complete, accurate, and current medical records in the individual file • Ensure documentation aligns with ODP 6400/6100, HIPAA, and agency policies • Support licensing inspections, QA&I submissions, and audits related to medical services • Track and submit required documentation to Supports Coordinators and funding entities Health & Safety Support:• Monitor chronic health conditions and ensure care plans are followed • Assist with development and updates to Health Care Plans, protocols, and risk mitigation strategies • Support incident reporting and follow-up related to medical or health concerns • Promote preventative care and wellness initiatives Collaboration & Communication:• Serve as a liaison between medical providers, families, guardians, Supports Coordinators, and internal teams • Educate DSPs and supervisors on health-related protocols and care needs • Participate in interdisciplinary team meetings and care planning discussions • Communicate medical updates to leadership as needed Qualifications Required:• High school diploma or equivalent • Minimum 2 years experience working in IDD, healthcare coordination, or residential services • Strong knowledge of ODP 6400/6100 regulations and medical documentation standards • Excellent organizational, communication, and follow-up skills • Ability to manage confidential health information (HIPAA compliant) • Valid driver's license and ability to travel between sites Preferred:• Associate's or Bachelor's degree in Health Services, Human Services, Nursing, or related field • Experience coordinating care for individuals with complex medical needs • Familiarity with EHR systems, incident management systems, and ODP reporting portals • CNA, LPN, or RN background (not required but strongly preferred) Knowledge, Skills & Abilities:• Strong attention to detail and documentation accuracy • Ability to prioritize multiple medical needs across several individuals • Professional communication with medical providers and stakeholders • Calm, solution-focused response in urgent or emergent situations • Commitment to person-centered care and dignity of risk principles Physical & Environmental Requirements:• Ability to travel to residential sites, hospitals, and medical offices • Ability to sit, stand, walk, and perform administrative duties • May be exposed to healthcare environments and emergency situations Performance Measures:• Timely completion of required medical appointments and follow-ups • Compliance with ODP medical documentation standards • Reduction in missed appointments and preventable health incidents • Positive collaboration with DSPs, families, and providers • Successful support during licensing inspections and audits Compensation: $20.00 per hour We are an equal opportunity employer and all qualified applicants will receive consideration for employment without regard to race, color, religion, sex, national origin, disability status, protected veteran status, or any other characteristic protected by law.
