Centurion Health contracts with state and local governments nationwide to provide comprehensive healthcare services to correctional facilities, state hospitals, and other community settings. We are seeking a full-time Transitional Healthcare Facilitator for needs at Pendleton Juvenile Correctional Facility in Pendleton, Indiana.Available Shift: full-time days, Monday-FridayPay Rate $25 per hour + benefits! The Transitional Healthcare Facilitator will be responsible for the development and implementation of transitional healthcare plans for incarcerated individuals scheduled for release. The Transitional Healthcare Facilitator will coordinate with the Indiana Department of Correction and other state and local agencies to ensure that pre-discharge planning and preparation is accomplished, and those re-entering society are provided with necessary services and resources. Short term travel will be required to visit other juvenile facilities within the state of Indiana.
