Our facility includes a 473-bed hospital, a 76-bed inpatient psychiatric unit in Westport, and a multispecialty provider group catering to diverse community needs. Job Summary: Deliver compassionate direct patient care to behavioral health patients according to facility policies and procedures. Conduct initial and ongoing bio-psychosocial assessments to inform the nursing care plan aligned with the master treatment plan. Collaborate in treatment team conferences to define and adjust goals tailored to each patient's age-related and condition-specific needs. Implement tailored nursing plans for assigned patients and facilitate group therapy sessions. Evaluate patients' responses to interventions and update care plans as necessary while coordinating with the treatment team. Perform thorough assessments to identify and address the unique physical, psychosocial, cultural, spiritual, and age-related needs of patients. Develop and document comprehensive nursing care plans alongside patients and their families to ensure holistic care delivery. Provide direct patient care in accordance with physician orders and established protocols, ensuring meticulous documentation. Recognize changes in patients' physical and psychological states to proactively adjust care plans. Delegate care tasks appropriately to other nursing staff to optimize patient care and team performance. Exhibit proficiency and skills required for interacting with behavioral health patients and collaborating with interdisciplinary teams. Perform additional duties as assigned while embodying the values of nursing ethics and standards. Qualifications: Associate Degree in Nursing (ASN) required; Bachelor of Science in Nursing (BSN) preferred. Demonstrated ability to manage challenging patient behaviors effectively as trained in MOAB. Must have a valid Registered Nurse license from the Connecticut Department of Public Health before hire or job transfer. Certification as a BLS Provider from the American Heart Association or American Red Cross must be obtained within 60 days of hire or transfer.
