RN Case Management Nurse

RN Case Management Nurse The RN Case Management Nurse functions with demonstrated expertise in the role as a nurse case manager for children, youth, young adults, adults, and pregnant women with HIV infection, infants born to women with HIV, and family members of patients. In addition: Has advanced knowledge and expertise in the understanding and implementation of the care needs of patients with HIV infection. Coordinates the care of patients across the continuum of healthcare settings including hospital, clinic, and the community. Collaborates with multiple providers working closely with the HIV specialty physician, HIV social worker, HIV multi-disciplinary care team, and community providers to assess needs, plan, and implement care to patients. Serves as a recognized expert in the care of patients with HIV and as a consultant to health care providers within CHW and the community. Education: Bachelor's degree in Nursing required Experience: 1 year nursing experience required. Experience in managing patients with chronic and complex health care needs preferred. Experience in the care of patients with HIV preferred. Licenses and Certifications: License to practice professional nursing in the state of Wisconsin. Current CPR certification 'Basic Life Support (BLS) for Healthcare Providers through American Heart or American Red Cross is required. Knowledge, Skills and Abilities: Compassion and commitment to caring for people living with poverty, mental illness, drug use, LGBTQ issues, and many complex social needs. Ability to work flexible hours to meet patient care needs. Analytical skills necessary to resolve complex patient problems, develop standards of care and procedures, and evaluation of the quality of nursing care. Interpersonal communication skills sufficient to interact effectively with clients and their families, other CHW departments, and interdisciplinary health team members within the community. Ability to communicate effectively via telephone, voice mail, e-mail Ability to organize work effectively and multitask.
