Certified Nursing Assistant (CNA) - Full-Time | Field-BasedVitality Hospice & Palliative Care Vitality Hospice & Palliative Care is a clinician-owned and operated hospice seeking a compassionate and dependable Certified Nursing Assistant (CNA) to join our growing interdisciplinary team. In this field-based role, you will provide hands-on care and emotional support to patients in their homes, helping preserve comfort, dignity, and quality of life during their end-of-life journey. Schedule & Position Details Full-Time, Monday-Friday Typical hours: 7:00 AM - 3:00 PM Occasional evening/weekend overtime available Field-based role with local travel Mileage reimbursement provided Hiring immediately Key Responsibilities Under the direction of a Registered Nurse Case Manager, the CNA will: Assist patients with mobility, transfers, and ambulation Provide personal care including bathing, grooming, dressing, and feeding Offer companionship and emotional support to patients and families Perform light housekeeping tasks related to patient care Accurately document care provided in a timely manner Follow individualized care plans and hospice standards Support a culture of compassion, respect, and patient-centered care Qualifications Active Certified Nursing Assistant (CNA) certification (required) Valid driver's license, reliable vehicle, and current auto insurance Ability to work independently in the field Strong communication, time-management, and organizational skills Compassionate, dependable, and patient-focused Experience with hospice, palliative care, or chronic illness preferred (not required) Why Join Vitality Hospice & Palliative Care?
