Job Summary You will utilize the nursing process (assessment, diagnosis, outcomes/planning, implementation, and evaluation) to provide personalized nursing care. Your responsibilities will include: Collecting and relaying information regarding patient care. Developing individualized patient education based on assessed needs. Implementing personalized care plans. Coordinating care across various settings by collaborating with the inter-professional team and community partners. Managing disease registries and following up with patients on clinical goals. Applying population health care management strategies to specified patient panels. We expect you to demonstrate competency within the full scope of nursing expertise and adhere to age and population-specific standards as outlined in your clinical setting. You will operate in accordance with the Nurse Practice Act and the Code of Ethics for Nurses. Qualifications Bachelor's degree in Nursing strongly preferred; required in specified locations. Graduate from an accredited nursing program. Minimum of two years of clinical experience preferred. Currently holds an unencumbered RN license. Basic Life Support (BLS) certification required within six months of employment. Valid driver's license is necessary.
