Home Health Physical Therapist Part Time BrightStar Care is seeking a compassionate and skilled Physical Therapist to join our home health team 23 days per week. This role involves providing personalized in-home therapy that helps clients maintain safety, health, and independence. We are proud to partner with the Veteran's Administration to serve Veterans throughout the Twin Cities and surrounding areas, and we are currently looking for a PT to support clients in St. Paul and the North Metro. Key Responsibilities: Assessment, planning, implementation, and evaluation of all physical therapy modalities to patients. All duties and standards will be performed according to established policies, procedures, and competency guidelines within the department and organization. Improves or minimizes residual physical disabilities of the patient. Returns the individual to optimum and productive level within patient's capabilities. Participates with all other home care personnel in patient care planning. Takes initial history and makes initial evaluation. Consults with physicians regarding patient's progress Participates in quality assessment and performance improvement activities as assigned. Prepares clinical and progress notes. Helps develop the plan of care and revise, as necessary. Consults with family and agency personnel. Ability to do extensive bending, lifting and standing on a regular basis. Requirements: A graduate of a PT Program recognized by the American Physical Therapy Association or other National accepted Council or Association An active license to practice Physical Therapy in MN A minimum of four (4) years' experience as a Physical Therapist, home care experience a plus Must have criminal background check Must have current CPR certification Must have active driver's license and clear driving record Computer skills. Ability to exercise initiative and independent judgment
