What We're Looking For We are seeking a passionate and caring individual with a background in nutrition or dietetics who wants to make a meaningful difference in the lives of older adults. As an Associate Care Manager at ElderTree, you will support the care management team in coordinating holistic care for our clients while bringing a unique nutritional perspective to help seniors thrive at home and in care settings. This is an ideal role for someone early in their career who is eager to grow in geriatric care management. We are looking for someone based in Northern Virginia, Washington DC, or Montgomery County, Maryland. Ability to drive is critical for the role. Responsibilities Nutritional Assessment & Support: Conduct nutritional assessments for clients and collaborate with the care team to integrate dietary recommendations into individualized care plans. Care Coordination: Assist senior care managers in overseeing and coordinating tailored care plans, ensuring clients receive appropriate and timely services. Client Advocacy: Serve as an advocate for clients, ensuring their nutritional and overall wellness needs are communicated to healthcare providers, families, and caregivers. Support & Collaboration: Work closely with team members across disciplines to address clients' needs and contribute your nutritional expertise to the team. On-Call Availability: Participate in after-hours or weekend on-call rotations as needed to provide continuous support to clients. Field Work & Documentation: Conduct in-person visits with clients, update family members and care team members, and maintain thorough and accurate documentation. Approximately 90% of the role involves field work, with 10% dedicated to follow-up and office-based tasks. Qualifications & Requirements Bachelor's degree in Nutrition, Dietetics, Food Science, or a related field Passion for working with older adults and improving their quality of life Available 9am to 6pm Monday–Friday and some weekends Dependable, adaptable, and accountable, with a strong commitment to providing exceptional care Must be able to lift and maneuver up to 50 pounds Must have a car with a good driving record and be comfortable driving regularly around the DC Metro area (tolls and mileage reimbursed) Preferred Qualifications Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential Certified Dietitian-Nutritionist (CDN) or state-equivalent license 1–3 years of experience in clinical nutrition, community nutrition, or a healthcare setting Experience working with older adults, home care, or geriatric populations Familiarity with nutrition-related challenges in aging, including dysphagia, malnutrition, diabetes management, and polypharmacy What We Offer Competitive salary commensurate with education and experience with additional bonus opportunities Health care paid for full-time team members 401K with company match Tolls and mileage reimbursement Education stipend Great team culture!
