Field Care Manager

Job Posting Title: Field Care Manager, Nurse 2 Location: Remote with 75% travel to an assigned area in Wayne or Macomb MI county Duration: 3 Months (Opportunity for ext. or Perm/ based off performance/evaluation and team needs/budget) PR: $40/hr. Required Qualifications • Active Michigan Registered Nurse (RN) license with no disciplinary action • Minimum of 2 years experience in health care and/or case management • Must reside in or near Wayne or Macomb MI county • Ability to travel to homes and community settings for face-to-face assessments • Experience working with the adult population, disease management. • Ability to work full-time (40 hours minimum) Mon-Fri • Valid driver's license, car insurance, and access to an automobile Preferred Qualifications • BSN • Experience with in home assessment and care coordination experience • Experience with health promotion, coaching and wellness • Experience with Medicaid Long Term Care • Previous managed care experience • Bilingual- Spanish, Arabic or Chaldean Neo-Aramaic
