Lead and collaborate with an interdisciplinary care team to deliver coordinated behavioral health services in the home. Serve as a trusted partner to clinicians, physicians, patients, and families ensuring clear communication and shared goals. Coach and support team members through regular feedback, problem-solving, and guidance. Promote teamwork and accountability while ensuring high clinical standards and timely patient care. Oversee plans of care with a focus on collaboration, continuity, and patient-centered outcomes. What We're Looking For Associate's Degree in Nursing (or related field) required 2+ years of home health or behavioral health experience as a Registered Nurse At least 1 year of leadership or team support experience Current state Registered Nurse license Comfort using Workday, HomeCare HomeBase, and MS Office tools We value the unique skills of veterans and military spouses.
