Coverage Area: Noble, LaGrange, Steuben, DeKalb & Allen Counties Setting: Home Health Position Overview We are seeking a compassionate and experienced Physical Therapist to provide skilled home health services across Noble, LaGrange, Steuben, DeKalb, and Allen counties. This role focuses on delivering patient-centered therapy to promote mobility, independence, and overall functional improvement within the home environment. Home care experience and proficiency with HCHB and OASIS documentation are required. Key Responsibilities Perform comprehensive physical therapy evaluations based on physician orders and referrals. Develop individualized, measurable treatment plans designed to restore function and maximize patient independence. Implement evidence-based exercise programs and rehabilitation interventions tailored to each patient’s needs. Reassess patient progress regularly and modify treatment goals as appropriate. Collaborate with physicians, nurses, interdisciplinary team members, patients, and family members to ensure coordinated care. Recommend, obtain, and train patients on adaptive equipment to improve safety and mobility. Maintain timely, accurate documentation in compliance with company standards, payer guidelines, and state/federal regulations (including OASIS requirements). Utilize HCHB (Homecare Homebase) for documentation and workflow management. Supervise Physical Therapist Assistants and therapy aides in accordance with state practice regulations. Participate in ongoing professional development and continuing education to maintain clinical excellence. Adhere to HIPAA, corporate compliance policies, and professional ethical standards. Required Qualifications Active Physical Therapist license in the state of practice Home health experience required Proficiency with HCHB and OASIS documentation Strong experience in exercise programming and rehabilitation Excellent communication and care coordination skills
