Principal Functions: The Quality Assurance (QA) Coordinator is responsible for the assessment, treatment, and education of WCCH patients and providing clinical support to Physicians and health care providers while working collaboratively with the Chief Executive Officer, (CEO), Medical Director/Physician, and the Chief Operating Officer (COO) to design, implement, and manage the Quality Improvement Program for WCCH. Essential Duties and Responsibilities: Oversees and monitors the development and implementation of the Quality Improvement Program for WCCH. Works with the COO to monitor patient activity related to quality assurance and address concerns. Reviews and evaluates patient health records, as needed, to assess quality care issues and quality indicators. Produces reports on identified quality indicator results and participates in data analysis. Develops and leads cross-functional teams in the development and implementation of improvement strategies. Manages the Chronic Care Collaborative project. Assists with staff training, development and clinical competencies for quality assurance. Develops and participates in training programs that focus on the components of the compliance and quality improvement programs. Collaborates with other agency staff in areas of quality improvement. Updates and revises policies and procedures as needed in support of quality and compliance initiatives. Presents updates and revisions of policies and procedures to the Leadership Team for approval. Works collaboratively with the Human Resources department and WCCH leadership to develop and monitor employee and patient safety and infection control programs. Consults and reports to practitioners, administration, Board and the Quality Improvement Committee on quality, performance improvement, and planned care model activities and goals. Reports information on activities of the compliance program to the CEO and Board Quality Improvement (QI), and Compliance Committee. Investigates any report or allegation of unethical or improper conduct or business practices. Monitors appropriate corrective action and /or subsequent compliance. Directs methods to reduce WCCH's vulnerability to fraud and abuse such as conducting periodic audits, developing effective lines of communication on compliance issues and preparing, maintaining, implementing and disseminating written practice standards and procedures. Responds to patient complaints, concerns, and questions in a manner that meets their physical and emotional needs. Works with HRSA and UDS team. Review medical records for accuracy. Organize patient data contained in medical records. Track and monitor HEDIS Program (Healthcare Effectiveness Data and Information Set) Consults with office staff or other healthcare professionals regarding patient care. Triages patients on the telephone and/or the internet. Assists with patients and provides education, counseling, case-management, and follow-up treatment. Assists with public relations programs within the community. Performs administrative tasks, to include completing forms, records, and patient charts; preparing and charting patient-care plans, scheduling appointments; obtaining prior authorizations from insurance companies for medications, testing, and office visits; phoning in prescriptions to pharmacists and ordering indigent medications for eligible patients; and assembling educational materials for community programs. Education and/or Experience - Associate of Science in Nursing (ASN) or Associate Degree of Nursing (ADN) or equivalent from a two-year college or technical school; or one-year related experience and/or training; or equivalent combination of education and experience is required. Bachelor's degree (B.A./B.S.) from a four-year college or university; or one to two-years related experience and/or training; or equivalent combination of education and experience is desired. A minimum of three years of previous experience in a health-related environment is required Additional required licenses and certifications include: Licensure and the ability to maintain a current Indiana license as a Registered Nurse (RN) Certification of the ability to obtain certification in Cardiopulmonary Resuscitation (CPR) LPN education consists of 12-18 months State approved Certificate or diploma program. Monday - Friday 8:00 am - 5:00 pm 1 hour lunch Vacation, Sick, and Personal Paid Holidays SALARY RANGE -- LPN $44,928 Annually RN $60,320 Annually
