Job Description

Statement of Purpose The Internal Medicine Advanced Practice Provider (APP) provides comprehensive, patient-centered primary care services to adult and geriatric populations in an outpatient practice setting. Under indirect supervision of a collaborating physician, the APP evaluates, diagnoses, treats, and manages acute and complex chronic medical conditions while supporting quality, access, and continuity of care consistent with Jones Memorial Hospital standards and goals. Essential Functions and Responsibilities Patient Care Conduct comprehensive patient assessments, including history, physical examination, diagnosis, treatment, and follow-up care for adult and older adult patients. Order, perform, and interpret diagnostic tests and develop evidence-based treatment plans for acute and chronic internal medicine conditions. Prescribe medications, therapies, immunizations, and preventive services in accordance with scope of practice and regulatory requirements. Provide patient education focused on chronic disease management, medication adherence, and preventive health. Monitor patient progress and modify treatment plans as clinically indicated, including management of multi-morbidity and medically complex patients. Collaborate with supervising/collaborating physician on complex, high-risk, or diagnostically uncertain cases and initiate referrals to specialists as appropriate. Meet established productivity, quality, and access expectations developed in collaboration with Practice Management. Quality, Compliance, and Risk Management Deliver care consistent with accepted national clinical standards and organizational policies. Maintain timely, accurate, and complete clinical documentation to support patient care, quality reporting, and billing requirements. Practice in a manner that supports regulatory compliance, patient safety, infection prevention, and risk mitigation. Actively participate in quality improvement initiatives, audits, and practice accreditation activities as applicable. Safeguard patient confidentiality and protected health information at all times. Practice and Resource Management Collaborate with physicians, APPs, nursing staff, and support staff to ensure efficient clinical workflows and optimal patient access. Participate in telephone call coverage and care coordination activities as assigned. Support cost-effective use of practice resources, including staffing, supplies, and clinical services. Leadership, Communication, and Culture Foster a collaborative, patient-centered practice environment aligned with health system values and goals. Demonstrate professionalism, accountability, and effective communication with patients, families, colleagues, and leadership. Participate in provider meetings and practice initiatives in support of clinical integration and continuous improvement. Community Engagement Serve as a clinical ambassador of the health system within the community. Participate in community outreach, education, mentoring, or committee activities as available and requested. Qualifications Required Graduate of an accredited Nurse Practitioner or Physician Assistant program. Current, unrestricted New York State NP or PA license. DEA registration and prescriptive authority (or eligibility to obtain). Demonstrated competence in outpatient internal medicine or adult primary care. Ability to work independently within scope while collaborating effectively with physicians and interdisciplinary teams. Preferred National board certification appropriate for adult primary care (e.g., Adult-Gerontology Primary Care NP, Adult NP, or PA-C with Internal Medicine experience). Experience managing complex adult and geriatric patient populations. Prior experience in outpatient Internal Medicine or hospital-affiliated primary care practices. Experience practicing in a rural or community health setting. Familiarity with quality-based or value-based care models. Additional Requirements Maintain all required licensure, certifications, registrations, and continuing education for employment. Commitment to timely documentation, regulatory compliance, and professional development. Ability to meet physical, cognitive, and communication demands of an outpatient clinical environment. Salary Range: $105,000 - $175,000. Placement within this range is based on the candidate's qualifications, experience, credentials, and demonstrated job-related capabilities.
