Job Description

Lead Medical Coordinator Serves as the primary liaison with health care providers to ensure clients receive comprehensive and timely medical care. Coordinates all aspects of client health, including primary care appointments, immunizations, medication administration, and ongoing medical needs. Ensures compliance with California Community Care Licensing (Title 22), Department of Health, and County Public Health Department requirements. Oversees medical documentation, monitors health outcomes, and provides guidance to staff on medical protocols to maintain a high standard of care. Ensure a safe, trauma-informed, and nurturing environment for clients. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Coordinate team based medical care for the children placed in the center. Prepare, compile, submit, and maintain accurate medical records, files, forms, statistics, and additional relevant information in accord with agency policy, licensing and/or funding requirements. Review ORR portal for compliance with medical information upload requirements and ensure updates are input as needed for safety and timely discharge. As appropriate, provide sponsors, family members and clients with medial updates. Actively participate in medical and service-related resource development prior to children's discharge. Participate in planning, coordinating, implementing, and evaluating all medical needs of the children for designated social service program(s). Establish and maintain partnership with medical care providers. Provide documented information related to identified delays in the medical assessments. Attend scheduled meetings and participate in team decisions and operations to represent the medical needs of the children. Coordinate medical resources for all employees and resource family homes. Adhere to Child Abuse and reporting requirements. Must be relied upon regarding task completion and meeting deadlines without sacrificing accuracy, work quality, or customer service satisfaction. Attending meetings and training when needed or requested. Perform other duties as requested by the assigned supervisor and/or management. Directly supervises employees in the Department. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Supervise, coaches, and mentors staff providing guidance to their work and learning including, when necessary, corrective/disciplinary action in a timely and professional manner. The requirements listed below are representatives of the knowledge, skill, and/or ability required. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential function. 2+ years working experience in child welfare. 1+ years of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care. AND Any of the following: Licensed Registered Nurse (RN) with a Bachelor's; OR Higher health-related qualification (e.g. Physician Assistant, Nurse Practitioner) Must be at least 21 years of age. Valid Driver License Licensed Registered Nurse (RN) with a bachelor's degree or a higher health-related qualification (such as a Physician Assistant or Nurse Practitioner) and two to four years' experience working with children and families. Bilingual in Spanish requested but not required. Read, analyze and interpret business, professional, technical or governmental documents. Write reports, business correspondence and procedure manuals. Effectively present information and respond to questions from managers, customers and the public. Comprehend and apply principles of advanced calculus, algebra and advanced statistical theory. Work with concepts such as limits, rings, quadratic and differential equations, and proofs of theorems. Apply logical thinking to a wide range of intellectual and practical problems. Dealing with nonverbal symbolism (e.g., formulas and equations) in difficult phases. Deal with a variety of abstract and concrete variables. Proficiency in MS Office (Word, Excel, Outlook) Proficient in the use of standard office equipment. Electronic Medical Record and UC Portal Software The noise level in the work environment is usually moderate. The employee is regularly required to sit. The employee is frequently required to use hands to finger, handle, or feel and talk or hear. The employee is occasionally required to stand and walk. The employee must be able to occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision.
