We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide on-call support for hospice and homecare patients in a regional healthcare setting within the Midwest. This part-time role involves responding to clinical and non-clinical situations after regular business hours, including weeknights and alternating weekends, covering 3 to 4 shifts weekly. Role Overview As an on-call LPN, you will deliver skilled nursing assessments, interventions, and supportive care aligned with individualized patient plans. You will collaborate closely with physicians, registered nurses, and other healthcare professionals to ensure optimal patient outcomes. Your responsibilities include triaging calls, making timely home or facility visits, managing medication administration, and providing education on symptom and pain management. Education Graduate of an accredited practical nursing program. Experience Several years of clinical nursing experience. Previous hospice or home health experience preferred. Licensure and Certifications Current LPN licensure valid in the state of practice. BLS certification required within three months of hire. Valid driver's license and auto insurance meeting regional requirements. Skills and Requirements Strong leadership and communication skills. Proficiency with MS Office and keyboarding skills. Knowledge of hospice and homecare regulations and standards. Ability to work autonomously with flexibility and accountability.
