This includes assessing, evaluating, and updating therapeutic/rehabilitative/functional status and participation in the development of the total plan of care. Responsibilities: Assess, evaluate, and update therapeutic/rehabilitative/functional status. Evaluate home environment for muscle strength, mobility, gait, ROM - potential for rehab. Instruct patients/clients and families/caregivers in therapeutic appliances and determine therapy priorities. Report patient's/client's reaction to treatments to the physician. Initiate physical therapy program and instruct in phases of therapy relevant to patient/client care. Supervise homemaker/Home Health Aides in rehabilitation regime activities. Identify needs for other home health services and make referrals as necessary. Prepare and submit clinical and progress summaries and documentation. Provide physical therapy consultation to home families/caregivers. Participate in inservice education programs and peer consultation process. Comply with relevant regulations and licensure requirements. Perform other duties as assigned. Requirements: 2 years experience in a healthcare setting required; community/home health preferred. Home Health experience including OASIS required. BLS and NPI certifications required. CA PT license required. Current driver's license & proof of automobile insurance required for HH. Good oral and written communication skills; good organizational skills. EMR HomeCare Home Base (HCHB) experience preferred.
