Duties Description Are you interested in starting a career with New York State? Capital District Psychiatric Center (CDPC) is recruiting an experienced Registered Nurse 2 Community. Candidates from outside or within State Service can apply. CDPC, a Joint Commission accredited Office of Mental Health (OMH) facility, provides inpatient and outpatient treatment and rehabilitation to adults who have been diagnosed with serious mental illness, outpatient treatment services for children and adolescents diagnosed with serious emotional disturbances, and operates adult community residences in the Albany/Capital District area. CDPC is committed to supporting underserved individuals, organizations, and communities along with activities and initiatives to reduce disparities in access, quality, and treatment for underserved populations. Under the direction of the Chief Nursing Officer, the Registered Nurse 2 Community will perform a variety of activities associated with the Skilled Nursing Facility Enhanced Support Program and provide nursing consultation, support, and oversight of the Family Care Program. Duties include: • Providing clinical intervention, education, planning, and guidance to individuals and their families transitioning into a Skilled Nursing Facility (SNF). • Conducting nursing assessments and developing individualized comprehensive plans of care and aiding in transition of individuals into SNF. • Developing relationships with skilled nursing facilities to present the Skilled Nursing Facility Enhanced Support Program with the goal of facilitating the discharge of patients who are no longer in need of inpatient psychiatric hospitalization. • Conducting monthly and as-needed visits to assigned SNF individuals to assess care and monitoring individuals mental and medical health needs and services. • Collecting, monitoring, and documenting details of support provided to individuals and submitting records monthly. • Reviewing and ensuring accuracy of individuals' medical documentation, medication orders, program participation, and treatment progress. • Preparing and providing clinical case presentations at University of Rochester Tele-ECHO clinics. Minimum Qualifications Candidates from outside State service may be considered for hire under the Hiring for Emergency Limited Placement - Statewide (HELPS) program. • License and current registration as a registered professional nurse in New York State; and • 2 years of post-licensure clinical nursing experience; • 1 year of this experience must have been providing direct services to individuals diagnosed with mental illness, developmental disabilities, addiction, or substance use disorders in a community setting. • Substitution: Bachelor's degree in nursing can substitute for 1 year of post-licensure clinical nursing experience; and a master's degree in nursing can substitute for 2 years of post-licensure clinical nursing experience. Neither degree substitutes for the required 1 year of specialized community nursing experience. OR Candidates from within State service may be considered for transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law. Additional Comments: Posted hiring rate includes the sum of the statutory hiring rate for PS&T Grade 20 ($75,193), an Increase Hiring Rate ($9,980), and Geographic Pay Differential ($20,000). The posted job rate is the sum of the statutory job rate ($94,121) Geographic Pay Differential ($20,000). Notes: 1.
