Thriving clinic environment, and providers see 12-18 patients per day. Conveniently located off Hwy 41 in the Fox Valley. Radiology, Lab, Physical therapy and Urgent Care onsite. EMR System: EPIC Facility: Ascension-Richmond St Clinic Location: Appleton, WI What You Will Do Ascension-Wisconsin is looking for a Board Certified/Board Eligible Advanced Practice Provider (Nurse Practitioner/Physician Assistant) for our Primary Care-Family Medicine Team. Orders, performs and interprets diagnostic studies. Performs preventative health assessments, screenings, immunizations and patient care. Provides direct treatment and management of health conditions via referral to other healthcare providers or community resources. Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources. Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy. What You Will Need Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required.
