This position is responsible for coordinating and facilitating nursing care activities to improve continuity of care across the continuum under the direction of the MHCC director for nursing care practice. Responsibilities: Coordinate and implement nursing services with other resident services to ensure continuity of care. Provide direct resident care and oversee case management. Act as a patient advocate and resource for patients, families, and staff. Provide consultative direction and expertise in nursing services. Complete, evaluate, and report monthly quality data. Develop and maintain departmental objectives and standards of nursing practice. Requirements: Graduate of an accredited nursing program. BSN required upon hire. 2-4 years of experience in a supervisory position in long-term care nursing. Licensed as an RN in the state of practice. Approved CPR certification required within 90 days.
