Job specification Candidate requirements Job specification • Lead a high-performing home care department by implementing and overseeing workflows, policies, and procedures to ensure consistent, high-quality care delivery. • Hire, train, mentor, and supervise home health staff including nurses and therapists, fostering a collaborative and professional work environment. • Evaluate the effectiveness of nursing assessments and care coordination through the Interdisciplinary Team (IDT). • Ensure timely and accurate delivery of nursing interventions as outlined in participant care plans. • Monitor and maintain documentation standards for all home care visits and services. • Oversee and evaluate the quality of care provided to participants in skilled nursing facilities and board and care homes. • Promote compliance with regulatory requirements and internal performance standards. • Collaborate with internal and external stakeholders to ensure optimal care transitions and patient outcomes. Candidate requirements • Registered Nurse (RN) License in the state of California - Required • Bachelor's Degree - Preferred (additional experience may be considered instead of degree) • Minimum of two (2) years of nursing experience in a home health setting with frail or elderly populations - Preferred • Strong understanding of geriatric nursing practices and complex clinical case management • Demonstrated leadership ability in a healthcare setting with proven coaching and team-building skills • Excellent interpersonal, organizational, and communication skills • Knowledge of federal and state regulations about home health care Why work with us?
