Psychiatric Mental Health Nurse Practitioner (PMHNP) Location: Maryland We are seeking a PMHNP to support adults with serious mental illness, co-occurring substance use disorders, and complex social needs. This role works across community-based behavioral health programs, including ACT teams and outpatient services. Care is delivered in clinics, homes, and community settings. ACT experience is highly preferred. Key Responsibilities Psychiatric Evaluations: Complete comprehensive assessments and mental status exams for new and established patients. Medication Management: Prescribe and monitor psychotropic medications; track response, side effects, and metabolic indicators. Interdisciplinary Collaboration: Coordinate with Case Managers, Social Workers, and medical staff to address clinical and social determinants of health. Crisis Response: Perform rapid risk assessments and stabilization during psychiatric emergencies. Specialized Care: Adjust treatment for transitional age youth, deaf/hard-of-hearing clients, and individuals involved in the justice system. Recovery-Focused Support: Educate patients and families on illness management and functional rehabilitation to support community integration.
