Home Care Physical Therapist Job Ref: 33737 Category: Physical Therapy Location: 133 Old Road to Nine Acre Corner, Concord, MA 01742 Department: Home Care Visits Schedule: Per Diem Shift: Day shift Hours: 8:00am-4:30pm Pay Range: $37.00 - $55.00 per hour Job Summary Reporting to the Rehabilitation Team Lead, the Physical Therapist is responsible for comprehensive patient management including evaluation, diagnosis, and intervention, and patient/family education for development and implementation of individualized care planning in the home setting. Opportunities include working directly in a multidisciplinary fashion, including specialty care groups such as Parkinson's Disease, Covid Recovery and Post-Acute Stroke care. The ideal candidate demonstrates strong clinical skills in Physical Therapy, with opportunity for clinical growth through education, in-services, and care management groups. This Home Care environment offers family-friendly, flexible hours. Minimum Qualifications Education Bachelors Degree or higher from an accredited physical therapy program. Experience Prior Home Care experience is preferred, or at least one year previous experience in an acute and/or rehab setting.
