Medication administration and assessments required.Job Dutieso Collaborate with healthcare providers, case management Agencies and Area on Agingo Daily assessments of clients. Monitor clients for signs and symptoms.o Document daily census and nursing assessments.o Communicate with clients, families and staff.o Intial assessments of new ADH clients.o Monthly assessmentso Facilitate the exchange of medical information between different healthcare providers as neededo This is not a hybrid position. Position is at the Adult Day Center.
