A progress note is written each visit. A re-evaluation is conducted when expected duration of treatment is reached. A summary is written upon patient’s discharge.Helps develop the plan of care and revises as necessaryPrepares clinical and progress notesAdvises and consults with the family and other Agency personnelPeriodically participates with all other health care personnel in patient care planningParticipates in in-service program
