Moreover, this position establishes a plan of care that fosters personal worth, spiritual well-being and human dignity in each patient. CORE JOB FUNCTIONS 1. Collaborates and facilitates patient discharge plans for those requiring post patient care. 2. Reassesses plan of care according to the physical and emotional needs of patient. 3. Performs patient assessment, including looking at illness, mental status, and family dynamics. 4. Documents relevant data accurately and in compliance with laws and regulations. 5. Links patients and their families to appropriate community resources. 6. Serves as patient advocate and liaison to physicians, insurance companies, and community agencies. 7. Helps the patient and their family to understand, accept, and follow medical recommendations. 8. Provides clinical supervision and guidance to junior social workers. 9.
