CV must have all dates in MM/YY to MM/YY format Work History must be in Sequential order with most recent on top Any Gaps over 30 days must be explained on the CV EPA (Please use the attached EPA. Its also in Trio under your Name & then click on Help) All Licenses, Boards and Certs with expiration dates must be listed on CV Anyone with malpractice must provide a NPDB Anyone with legal issues must provide supporting documents Availability 1st possible date they would be available? How many shifts would they be able for per month? What month is their schedule completely open? Any shift preferences (Day, Night, Blocksetc)? Any dates in the next 3-6 months they cant work? Any Holidays in the next 3-6 months they would be open to work.?
