Job OpportunityJob ID:52495 Sign On Bonus Available, Complimentary Medbridge Subscription & Free CEU Courses Description: Positions Location: Charlotte, MIJob Description General Purpose of Job: Develops and implements physical therapy treatment plans for assigned patients; evaluates and modifies patient care treatments as appropriate. Provides physical therapy interventions in order to maximize the values, potential, and satisfaction of our patients. Assumes responsibility for creating experience-based service..Essential Duties:Completes initial physical therapy evaluations and develops a treatment plan of care for assigned patients; coordinates acquisition of needed adaptive devices, durable medical equipment and orthoses.Provides physical therapy treatment including gait training, transfer training, balance training, strengthening, stretching and other modalities.Documents patient treatments, initial evaluation, progress notes and discharge summaries in a timely manner and in accordance with Hospital policies and insurance requirements.Performs physical therapy re-evaluations to assess patient progress; recommends changes to the treatment plan of care as appropriate.Communicates regularly with physicians, interdisciplinary team members, patient families, insurance companies and others regarding patient's treatment and progress.Instructs and teaches patients in safe transfers, activities of daily living, correct ambulation, therapeutic exercises and correct usage of exercise equipment; trains family members and caregivers on various therapeutic procedures and home exercises.Provides guidance to assigned staff. Duties include, but are not limited to, assigning and reviewing work, training, co-signing documentation. Provides supervision to PTAs and Rehab Aides.Schedules patient treatments/appointments along with referring patients to other hospital services.Attends staff meetings and in-service programs as required.Assists in maintaining therapy equipment and treatment areas; orders supplies as necessary.Supports and serves as a role model for our mission, vision, values, and customer service initiatives. Adheres to the organization's policies & procedures, and compliance guidelines.Is responsible for creating a destination, experience-based service; delivers customized experiences using patient profiles.Performs other duties as assigned. Job RequirementsGeneral Requirements Valid State of Michigan Physical Therapist LicenseCPR/BCLS certificationValid Michigan driver's licenseProof of current Automobile InsuranceWork Experience Minimum of one year of related physical therapy experience or school-based experienceEducation Graduate of an accredited school of Physical TherapySpecialized Knowledge and Skills Knowledge of physical therapy theory and techniquesKnowledge of accrediting and regulatory standards and hospital policies related to the provision of rehabilitation services including HIPAA confidentiality and JCAHO mandatesWritten and computer skills necessary to prepare required documentationInterpersonal skills necessary to effectively communicate, orally and in written form, with patients, families, physicians, health care professionals, and insurance companies in the exchange of information. Presents self in a warm, professional and caring manner.Demonstrates professional appearance.Skills necessary to provide guidance, training, prioritize and review the work of assigned staffAnalytical ability to develop treatment plans based on physical therapy theory and incorporation of patient and physician goalsMental ability to complete required documentation in a timely manner, deal with the concerns of patients and their families, and handle frequent scheduling changesPhysical ability to stand for prolonged periods of time, transfer/lift/ reposition patients, assist patients with exercises, properly operate rehabilitation equipment/devices, and access patient homes which may not be barrier freeUMH-Sparrow therapists enjoy strong professional development support, including a complimentary Medbridge subscription that provides access to a wide range of continuing education courses.
