About the Role The RN Case Manager plays a central leadership role in our hospice model of care. From admission through the end of the patient's journey, the RN Case Manager provides comprehensive professional nursing care utilizing all elements of the nursing process. This position leads a designated interdisciplinary team and manages a personal territory throughout Orange and Sullivan Counties. Key Responsibilities Conduct comprehensive assessments and ongoing evaluations Develop, implement, and revise individualized plans of care Provide hands-on skilled nursing care in patients' homes Lead and coordinate care with an assigned LPN and Hospice Aide Specialist Collaborate closely with Social Work and Spiritual Care to ensure holistic care Educate and support patients and families throughout the hospice experience Participate in interdisciplinary team meetings Schedule Monday - Friday 8:30 a.m. - 4:30 p.m. Participation in a rotating weeknight on-call schedule required RequirementsWho are we looking for? Graduate of an accredited school of Nursing with current and valid NYS Registered Nurse license is required; BSN preferred 3-5 years recent clinical nursing experience with minimum 2 years medical surgical experience in an institutional setting, home care or skilled nursing facility preferred Hospice admission and end-of-life care experience preferred Commits to take and pass the hospice and palliative care nursing certification exam (CHPN) within three years of hire if not currently certified Must have excellent written and verbal communication skills Proficient in MS Office Suite, EMRs and ability to learn new software programs Must have daily access to a vehicle, valid driver's license, acceptable driving record, and auto insurance to travel to patient locations Commitment to the HOSC mission and philosophy of care Ability to relate to a diverse population and workforce in a personable and effective manner Able to communicate effectively, while sustaining a caring, kind, and professional disposition
