Collaborate with the interdisciplinary team to support resident rights, well-being, and understanding of the social services role. Participate in developing individualized, interdisciplinary care plans addressing each resident's social service needs. Conduct meetings with residents and their support systems to provide guidance, resources, and ongoing support to enhance quality of life. Manage discharge planning, including education, documentation, and referrals to community agencies and service providers. Build and maintain positive relationships with community-based organizations to support effective referrals and resource coordination. Qualifications Bachelor's degree in Social Work or a related human services field (e.g., sociology, gerontology, special education, rehabilitation counseling, psychology). At least one year of supervised social work experience in a healthcare setting working directly with individuals. Required state licensure/certification to practice in a long-term care environment.
