Physical Therapist (PT) – Part-Time / Per-Diem – Home HealthCareSense Home Care| Phone: (888) 444-8157 | Email: Position Summary:CareSense Home Care is seeking a licensed Physical Therapist (PT) to provide home-based therapy services on a part-time or per-diem basis. The PT will conduct evaluations and deliver skilled interventions to adult and elderly individuals enrolled in Medicaid Waiver (OBRA, Act 150), Community HealthChoices (CHC), and private insurance programs. This is an excellent opportunity for clinicians seeking flexibility, autonomy, and meaningful work in a community-based setting.Essential Responsibilities: Conduct initial assessments and routine re-evaluations in participants’ homes.Develop and implement individualized therapy plans in alignment with the participant’s physician orders and care goals.Provide therapeutic services focused on mobility, balance, strength, and fall prevention.Educate participants and caregivers on home exercise programs and safety techniques.Complete all documentation per Electronic Visit Verification (EVV) and MCO standards using the agency’s EMR (e.g., HHAeXchange).Communicate effectively with the interdisciplinary team, including nurses, aides, and case managers.Participate in case reviews or virtual team meetings as needed.
