About the Role: Job Summary The ADT Nurse (admission, discharge, and transfer) is a registered nurse who is primarily responsible for consulting and advising on assessments and documentation for arrivals and departures on our identified targeted units as directed by the Capacity Command Center (Care Traffic Control). The ADT Nurse will assist with admission assessments, discharge instructions, reduce discharge barriers, and facilitate patient transfers off of the floor to ensure timely transfers and patient satisfaction. The ADT RN communicates with appropriate physicians, care management, CTC staff, nursing unit staff, and initiates necessary interventions to expedite the discharge process. Is a professional member of VOICE providing feedback on issues being addressed at meetings. Participates as a member on VOICE councils as appropriate. Actively participates on unit practice council helping with plans to improve NDNQI nursing sensitive indicators and Patient Experience data. Works together with other care team members to recruit and retain an excellent nursing care team. Minimum Job Qualifications Licensure or other certifications: Licensed to practice as an RN in Georgia. Educational Requirements: Associates Degree in Nursing. Minimum Experience: A minimum of three (3) years' recent broad adult acute care experience across med surg and telemetry level of care units. Other: Preferred Job Qualifications Preferred Licensure or other certifications: National certification in clinical specialty. Preferred Educational Requirements: Bachelor of Nursing. Preferred Experience: Other: Membership in a professional nursing organization. Job Specific and Unique Knowledge, Skills and Abilities Demonstrates the ability to collaborate with nursing staff, nursing leaders, care management, and physicians to facilitate the safest disposition for the patient. Demonstrated ability to plan, organize, direct, and evaluate activities related to patient care functions. Demonstrates appropriate clinical and technical skills. Demonstrates consistent ability to utilize appropriate critical thinking skills in assessments and interventions as evidenced by verbal or written reports to nursing administration. Essential Tasks and Responsibilities Responds to nursing units as directed by Capacity Command Center staff to address patient discharges and admissions depending on demand throughout the shift. Collaborates with nursing staff and physician reduce barriers and improve timeliness of the discharge process. Assists with the necessary interventions and/or transfers. Completes documentation of event, collaborating with the care team as needed. Coordinates discharge planning and prioritization across multiple units. Provides effective discharge education appropriate for the learning needs of patient/ family. Is an active participant in Performance Improvement processes related to the discharge and admission process. Collaborates with NGMC Medical Transport to assure appropriate allocation of resources to assure safe transport. Maintains effective working relationships with nursing staff, medical staff, and/or other hospital departments. Consistently communicates with care team members and Care Traffic Control concerning ADT events. Initiates, directs, and participates in planned change to improve patient care and/or operations, and customer service. Supports the goals and vision of the capacity command center. Provides patient care, applying advance clinical skills and critical thinking to ensure optimal clinical outcomes. Provides support and acts as a clinical resource for staff: monitor clinical practice to ensure compliance with established standards. Nursing assessment is focused to target areas most productive for a given patient population or situation. Recognizes subtle changes in clinical situation; evaluates and appropriately alters the plan of care. Is accountable for prioritizing and organizing patient care and ensuring follow through with planned care. Performs other duties as assigned.
