Duties: Assist new residents with move-in processes, including lease signing, obtaining furnishings for their home if needed, etc. Complete program-specific initial and annual paperwork with residents to assist with housing stability. This includes completing a needs assessment and housing service plan with residents. Develop positive relationships with collaborative partners, external providers, and Nesika Illahee property management. Be responsive to property managers as concerns arise to support residents to successfully maintain their housing and prevent eviction. Follow up with property managers to determine outcome of any needed interventions. Work in close coordination with other providers and support systems to address the needs of residents. Includes initiating clinical consults and assuring coordination of treatment planning and ongoing support services, including employment readiness. Provide home and community-based engagement services to clients utilizing strengths-based case management. Utilize assertive engagement techniques to meet residents where they are willing to participate and coach residents in setting goals and identifying realistic steps to achieve them. Coordinate cultural activities and supportive programming for all residents, in collaboration with project partners. This may include facilitating activity groups or support groups. Consistently use strong clinical and professional judgment while providing services to residents, including taking appropriate safety precautions while transporting residents and providing services to residents in the community and/or their homes. Demonstrate responsibility for safety of residents and property. Must be familiar with fire and building regulations and evacuation procedures. Provide ongoing assessment of needs and risk of harm for each individual and respond appropriately. Complete housing-related service plans, progress notes, discharge summaries and other documentation as needed. Qualifications: Qualified Mental Health Associate (QMHA) or Certified Recovery Mentor (CRM) credential required. QMHP strongly preferred. CADC or equivalent highly desirable. Professional experience working with individuals in recovery from a substance use disorder. Knowledge of local social service resources and programs. Knowledge and understanding of recovery-based, holistic approaches to addressing behavioral health needs. Knowledge and skill in working in a multi-cultural and multi-disciplinary environment. Must have valid driver's license and personal vehicle. Ability to pass a pre-employment or for cause drug tests. Ability to pass criminal background and DMV checks. Must be able to pass a CRIMS finger printing and background check. Either High School Diploma or General Educational Development (GED) certification required.
