Responsibilities Home health setting providing post-acute care Patient population includes those requiring rehabilitation and active therapy in their home environment Expectations include evaluation of home safety and mobility, as well as use of therapeutic appliances Uses Homecare Homebase (HCHB) for EMR documentation; experience preferred Standard schedule is 8-hour day shifts with every other weekend required 13-week assignment with 8-hour shifts Billable mileage per GSA guidelines; personal timesheets must be approved by supervisors Primary duties include assessment, implementation, and reassessment of physical therapy treatment plans as ordered by a physician Responsible for evaluating home environment and educating patients, caregivers, and families May provide instruction on prosthetic device use and supervise Home Health Aides in rehabilitation tasks Prepares and submits clinical and progress summaries according to policy and regulatory requirements Participates in peer consultations and ongoing staff competency assessments Candidates must not have been employed directly (FT, PT, or PRN) by any facility associated with CommonSpirit, including CHI and Dignity, within the past year; PRN through outside agencies is acceptable Attendance at bi-monthly meetings for timesheet approval may be required Open to travelers with previous community or home health experience Requirements Current and active CA Physical Therapist license required if applicable Minimum 2 years of physical therapy experience in a healthcare setting required Home Health and OASIS experience required Current BLS certification and NPI number required Valid driver's license and proof of automobile insurance required Strong organizational as well as oral and written communication skills needed 2 professional references required: one from a supervisor within the last 12 months, and one peer or supervisor within the last 3 years Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts American Traveler Job ID #P-673605.
