If interested in this RN Case Manager - Hospice position APPLY NOW for IMMEDIATE consideration Alexis Wordlaw-Conley| | text 5864221748 RN CASE MANAGER - Hospice| DETAILS AND COMPENSATION: Location: Camp Hill PA 17011 – 100% On-site, field work Coverage are : Dauphin, Cumberland, Perry, and Lebanon counties Payrate: $56.14/hr + mileageRequired Availability: Full-Time | Monday - Friday 8:00am to 5:00pm; no weekend or on-call RN CASE MANAGER - Hospice| RESPONSIBILITIES: Perform comprehensive assessments of high-risk membersCollaborate with primary care providers to implement individualized care plansIdentify and continuously reassess appropriate and cost-effective levels of careCoordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionalsArrange non-medical support services (e.g., housing, transportation) to support treatment complianceEngage specialty resources as needed to resolve complex care needsMaintain detailed documentation of clinical, functional, and fiscal outcomesIdentify opportunities for health promotion and illness preventionPrevent adverse patient events when possible and intervene promptly when necessaryAct as a patient advocate while ensuring privacy and confidentiality RN CASE MANAGER - Hospice | REQUIREMENTS: Education: Associate’s or Bachelor’s Degree in Nursing (or related field)Attendance: Commitment to mandatory attendance during the first 90 days.Active, unrestricted Registered Nurse licenseCase Management certification preferredStrong clinical assessment and care coordination skillsAbility to analyze complex medical information and make sound clinical decisionsExperience in case management or community-based care preferredHome health experience is a plusHospice and HCHB experience required.
