Role Overview We are actively looking for Primary Care Nurse Practitioners to deliver on-site care to residents in Assisted Living and Residential Care Facilities across various communities. There are approximately 300 eligible residents ready for care, ensuring a steady and predictable volume of work. What You'll Do Conduct routine and urgent assessments, as well as manage chronic diseases. Perform post-discharge follow-ups, medication reconciliation, and labs/diagnostics. Collaborate effectively with facility staff, specialists, and residents' families or Power of Attorneys. Complete same-day documentation while ensuring compliance with CMS and regulatory requirements. Schedule & Setting Part-time positions are available during weekdays, with clustered routes to minimize travel time. Requirements Qualifications Must possess active CA NP and RN licenses, an active furnishing license, NPI, DEA, and BLS certification. Experience in geriatrics or facility-based settings is preferred; proficiency in EMR is essential. Benefits Compensation (California pay transparency) The annual compensation ranges from $150,000 to $200,000, depending on experience, work schedule, and patient volume. Additional earning opportunities are available based on coverage and panel needs. This is a W-2 position with benefits offered, and mileage will be reimbursed in accordance with policy.
