Join us and helpcreate a place where residents—and teammates—thrive.Position SummaryThe Licensed Practical Nurse (LPN) delivers high-quality, person-centered nursing care to residents in Assisted Living and Memory Care. This role performs assessments within LPN scope, administers medications and treatments, documents accurately in the EHR, and collaborates with the Director of Health & Wellness/Memory Care and interdisciplinary partners to support resident well-being, safety, and dignity. The LPN provides clinical guidance to caregivers/med techs during assigned shifts and responds promptly to resident changes in condition. Perform focused nursing assessments within LPN scope; obtain and document vitals, pain levels, and condition changes; escalate per protocol.Administer medications and treatments (e.g., wound care) accurately and timely; maintain MAR/TAR per policy.Support development and ongoing review of individualized service/care plans; implement interventions and document outcomes.Monitor for adverse reactions, falls, elopement risk, hydration/nutrition status, skin integrity, and infection-control concerns; initiate first response and notify leadership/providers as required.Document care, assessments, and resident responses in the EHR in real time; complete shift reports and hand-offs.Communicate promptly with the DON/Memory Care Director, providers, and families regarding changes in condition, new orders, and follow-up needs.Provide on-shift direction to caregivers/med techs; reinforce best practices in dementia-capable care, ADLs, and safe transfers.Delegate and verify tasks consistent with the IL Nurse Practice Act and company policy; conduct spot checks for med pass accuracy.Follow infection-prevention protocols, universal precautions, and medication management policies.Complete incident reporting, root-cause details, and corrective actions; participate in audits, QA, and mock surveys.Uphold resident rights, confidentiality (HIPAA), and company standards of service excellence.Educate residents/families on care plans, disease processes (including dementia), and medication regimens; address questions with empathy.Coordinate external appointments, labs, and home health/hospice per orders; ensure proper documentation.Act as first clinical responder for urgent needs; initiate EMS per protocol; provide accurate hand-off to higher level of care.Precept new staff; participate in in-service training and competencies.Perform other duties as assigned by leadership. Qualifications (Required) Current, unencumbered Illinois LPN license.Graduate of an accredited practical nursing program.Current CPR/BLS certification.Proficiency with electronic health records (e.g., PointClickCare, MatrixCare, Eldermark) and basic computer skills.Ability to read, write, speak, and document in English; strong clinical judgment, organization, and communication skills.Ability to stand/walk most of shift; safely lift/pivot up to 50 lbs with assistance; perform safe transfers.Successful background check, drug screen (as applicable), TB screening, and required immunizations.
