Job Details POSITION SUMMARY - RN Case Manager POSITION DUTIES - Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans / Resource Utilization MINIMUM REQUIRED QUALIFICATIONS - CA RN license AHA BLS 2+ years recent RN Case Management experience, with experience in a LTAC or STACH setting, MCG, discharge planning across acute care settings, strong IDT collaboration LENGTH OF ASSIGNMENT - 13 weeks SHIFT / HOURS PER WEEK - M-F 8a-430p. Weekends on request.
