The position provides direction and leadership to ensure DePaul meets the Quintuple Aim of enhanced patient care, improved population health, reduction of cost, better care team well-being, and advanced health equity. This work includes establishing, meeting, and continuously monitoring the metrics in DePaul's Value Based Contracts and Population Health initiatives, while maintaining alignment with quality goals and objectives for the DePaul Clinics. The Director of Population Health works closely with a variety of stakeholders, coordinating the activities of Population Health across the enterprise to develop, implement, and evaluate Population Health programs and initiatives. The incumbent will direct and maintain the Population Health Roadmap, which encompasses the Population Health Care Model and Community Health Improvement plans. This leader serves as the programmatic and operational leader for Population Health and is responsible to work with the CPIACO & the Chief Nursing Officer (CNO) to align new value-based contracts with the population health & quality initiatives. The Director will work in a collaborative relationship with the CPIACO & the CNO, and work closely with DePaul employees, leaders, and cross-functional teams across the enterprise. Essential Responsibilities The following responsibilities are not intended to describe every function of this role. Develops and manages operational initiatives with measureable outcomes (in department goals, contract metrics, team productivity, etc) Actively seeks opportunities to improve financial outcomes from VB Contracts and aligns population health personnel to achieve those targeted outcomes Formulates objectives & goals with the CPIACO, the COO & the CNO to align population improvements in care with the financial benefits from VB contracts & Clinic Operations Integrates all Population Health work with clinic operations, to improve the overall health of the populations managed Works with the planning resources to guide them through FTE analyses and make recommendations for staffing enhancements that will drive improved financial outcomes in VB contracts and align programs with improved quality of care Works with the CNO to engage staff and other stakeholders in continuous improvement activities associated with organizational systems, improved quality outcomes and improved clinic processes (as it pertains to managing populations & improving contract outcomes) Enhances the current population health management team functions and expands team services to support enhanced outcomes in populations managed Perform other related duties as required Qualifications: Bachelor's degree in Health Administration, Nursing, or similar other degree Three to five years relevant experience in managing population health-related programs in environments like clinical practices, health systems, health plans or public health Track record of achievement in improving quality outcomes while decreasing the cost of care associated with value based contracts Demonstrated skill in communicating with and relating to physicians, nurse practitioners, operational leaders and other leadership at all levels within an organization, including the ability to establish a cohesive partnership with stakeholders Ability to function independently, manage multiple projects, and operate a team to deliver on value-based contract metrics (quality & cost) Works with providers to accurately reflect disease burden of Medicare beneficiaries across the enterprise; partners with health plan to improve RAF data submissions Uses data to drive decisions, plan, and implement performance improvement strategies for care coordination
