Department: Home Health / Hospice RNCertifications Required: BLSLocal Rate: Candidates living within 80 miles of the facility. Client rates reduced by XX8.00/hour.- Common diagnosis / Types of patients/patient demographics: No Peds. Home care & hospice for primarily Geriatric population: Heart Disease, Chronic Respiratory Diseases, CHF, COPD, Pneumonia, Dementia/Alzheimers, Depression, Stroke, Cancer, Kidney Disease, Diabetes, Liver Disease, Wounds, Joint Replacement, Surgery, Medication Management. - Additional Job Duties/Utilization Review involved?: Daily 15 minute Huddle Call, weekly IDT meeting at office, monthly all staff meeting. - RT Equipment: No vents. Cpap & BiPap on occasion - patients self manage. - Experience or 'Must have' skills: Travel experience, Needs expereince with OASIS. Critical Thinking Abilities, Chronic Disease Management, Patient/Family Education, NPWT, Ostomy Care, Central Line Care, IV Administration, Complex Wound Care. Hospice Experience: Pain & symptom management, end-of-life care, empathy, compassion. - How many patients do they see per day?: Will see 5-6 patients per day depending on type of visit, 25-30 points per week, will be routine & OASIS visits. - Manager/Supervisor: Administrator always available after hours/weekends/holidays. - Home Health/Hospice Aids?: We have 3 certified nursing assistants. - Shifts & Scheduling: Monday thru Friday 8-4:30 (end time may vary). - Weekend Requirements: Weekends only required during on call shift. - Holiday Requirements: Must be available to work at least 1 major holiday & 1 minor holiday. - Is there a call or standby requirement?: 1 on call week/weekend every 6 weeks (on call days do not have to be consecutive). - Floating Requirements: No floating, but YES will do both home health & hospice visits. - Scrub Color/Dress Code: Solid color scrubs (no patterns or logos unless company-issued). Closed-toe, non-slip shoes with socks or hosiery. No flip-flops, open toe shoes, or sandals allowed in any patient care area. - Unit/Department specific orientation: 1 day computer-based training, 2-5 days (depending on experience) of field training/shadowing to learn dept specific procedures.MUST note in submission if clinician has ever worked for Marshfield. If clinician previously worked with Marshfield, they must have an end date 6 months prior to their submission date.All clinicians must clock in and out at facility Date Entered On: 11/1/2025
