Job Overview Join our passionate team as a Registered Nurse (RN) Care Coordinator. In this crucial role, you will be instrumental in supporting our Medicaid and Medicare members. You'll conduct thorough assessments, develop customized care plans, educate members, and connect them with vital community resources. Your strong computer skills and keen attention to detail will be essential for managing various systems, facilitating seamless communication with members and providers, and ensuring precise documentation. This position is fast-paced, emphasizing both productivity and top-notch quality of care. Work Environment This is a remote position, requiring high-speed internet access. You will work Monday to Friday from 8:00 AM to 5:00 PM EST, with some flexibility to extend until 6:00 PM on one or two evenings each week. There are no weekend or holiday shifts. Key Responsibilities Conduct comprehensive assessments to determine member eligibility for care management based on clinical judgment and identified needs. Create and implement collaborative care coordination plans tailored to meet individual member goals. Engage in telephonic or in-person visits to monitor care plans on an ongoing basis. Facilitate integration of services across behavioral health, long-term support services, and community resources. Lead interdisciplinary care team meetings to foster collaboration among healthcare professionals. Utilize motivational interviewing techniques to empower and educate members on their care journey. Identify and address barriers to care, providing necessary support to members. Manage complex member cases and conduct medication reconciliation as required. Maintain regular outreach for continued member support and effective case management. Required Qualifications Minimum of 2 years of healthcare experience, ideally in care management or a related medical/behavioral health environment. Active and unrestricted RN license. Valid driver's license and reliable transportation for job-related travel. Familiarity with electronic medical records (EMR) and HIPAA regulations. Established knowledge of community resources available to members. Proficient in Microsoft Office and skilled at navigating online databases. Exceptional verbal and written communication skills, with a focus on problem-solving and critical thinking.
