Join a dedicated healthcare team providing compassionate home health and hospice care across the Midwest region. As a Homecare Occupational Therapist (OT), you will develop personalized care plans following thorough patient evaluations to meet individual and physician goals. You may be assigned to home health or hospice patients and will collaborate closely with interdisciplinary teams to ensure comprehensive care. Essential Duties and Responsibilities Evaluate patients for home health admission based on CMS, payer guidelines, and agency policies. Conduct initial and comprehensive assessments within required timeframes, documenting findings accurately. Create individualized, age-appropriate care plans focused on self-management with measurable goals and interventions. Update care plans promptly to address new or resolved issues. Communicate patient progress and any emerging concerns with physicians and care team members. Include patients and caregivers in care and discharge planning, tailoring education to their learning preferences and barriers. Manage care delivery efficiently, including scheduling visits and utilizing supplies cost-effectively. Complete all documentation and revisions within designated timeframes. Supervise Certified Occupational Therapy Assistants (COTA) and homecare assistants as applicable, ensuring alignment with care plans. Collaborate with multidisciplinary teams through meetings and direct communication to optimize patient outcomes. Assist patients in obtaining necessary equipment and begin discharge planning upon admission. Provide treatments per physician orders, adhering to infection control and hand hygiene protocols. Education Graduation from an accredited Occupational Therapy program is required. Experience At least one year of experience in a JCAHO-accredited hospital. Minimum one year in an acute rehabilitation setting or equivalent, serving adult patients. Preferred: Six months of home care experience. Computer skills are preferred. Certification and Licenses Current certification from the American Occupational Therapy Association (AOTA). Valid Occupational Therapist license in the state of Missouri. American Heart Association CPR certification. Current driver's license and automobile liability insurance. Mental and Physical Requirements Ability to stand and walk for extended periods and navigate stairs. Strong communication skills, both oral and written. Excellent interpersonal and organizational abilities. Safe automobile operation with 2-4 hours of daily driving. Capability to maneuver around home architectural barriers and tolerate environmental factors such as temperature changes, smoke, fumes, dust, and pets. Working Conditions Exposure to communicable diseases, blood, and body fluids. Potential contact with infectious diseases and occasionally hostile individuals. Handling of sharps and biohazardous materials. Varied environmental conditions across community settings.
