Job Title: Physical Therapist Assistant (PTA) - Skilled Nursing Facility Overview: We are seeking a compassionate and motivated Physical Therapist Assistant (PTA) to join our rehabilitation team at our Skilled Nursing Facility. In this role, you will assist in providing high-quality therapy services under the supervision of a licensed Physical Therapist, following the established plan of care to help residents regain strength, mobility, and independence. Your expertise will support the health and wellness of our residents, while your ability to motivate and educate will play a vital role in their rehabilitation journey. Key Responsibilities: Assist the Physical Therapist in the development and implementation of treatment plans, in accordance with state regulations. Provide direct treatment, including therapeutic exercises, massage, and modality-based interventions. Instruct and support patients during therapeutic activities, ensuring safety and encouragement. Monitor and document patient progress, updating the Physical Therapist through clear and timely reporting. Collaborate with the therapy team and other clinical staff to coordinate and adjust care plans as needed. Educate residents, caregivers, and family members on home exercise programs and therapy goals. Transport, transfer, and position patients safely using proper techniques and equipment. Ensure proper use and maintenance of therapy equipment, documenting treatment and equipment use as required. Measure and record range of motion, vital signs, and other clinical data relevant to treatment outcomes. Assist with fitting patients for orthotics, prosthetics, and supportive/assistive devices. Maintain compliance with all facility, state, and federal regulations and standards. Perform other related duties as assigned. Qualifications: Associate's degree from an accredited Physical Therapist Assistant program Current state licensure from the Board of Physical Therapy Examiners Prior experience in a Skilled Nursing Facility (SNF) preferred Knowledge of geriatric care and rehabilitation practices Strong understanding of state, federal, and professional therapy standards Excellent communication, organization, and documentation skills Ability to work collaboratively in a multidisciplinary team environment Compassionate, patient-centered approach with strong customer service skills Ability to adapt to changing patient needs and therapy plans Physical Requirements: Ability to lift a minimum of 20 lbs Frequent standing, walking, pushing, pulling, reaching, crouching, bending, and transferring patients Use of hands for typing, writing, and operating therapy equipment Join a team that values excellence in care and supports your professional growth in a collaborative environment.
