Join us as a Palliative Care Nurse Practitioner to provide patients and their families with the support they need during challenging times. As a Palliative Care Nurse Practitioner, you will: Collaborate closely with the Palliative Care Medical Director and team to ensure comprehensive palliative care for patients. Conduct thorough assessments including disease impacts, functional changes, and symptom management, ensuring a holistic approach to care. Develop customized care plans in consultation with patients and their families to meet specific needs and preferences. Provide accessible care and support when needs arise, ensuring smooth transitions throughout the palliative care process. Participate in on-call services as required, delivering responsive and competent care to those who need it most. Prescribe necessary medications and order diagnostic tests, always acting within the scope of your practice and state law. Regularly communicate clinical findings and updates with the Palliative Medical Director, ensuring a high standard of care. Foster collaborative relationships with supportive care team members to enhance patient care. Maintain accurate documentation of clinical findings and treatment plans to support effective practice and reimbursement. Stay informed about Palliative Medicine Quality Measures and ensure proper documentation and reporting. Are you the right fit? Requirements: Exceptional leadership and communication skills, both written and verbal. Ability to make independent decisions and display self-motivation in practice. Understanding of the principles of Palliative Medicine and related regulations. Must comply with continuing education requirements of Gentiva and the licensing board. Proficiency in point-of-care software is essential. Education and Experience: Master's degree in Nursing from an accredited program or equivalent as specified by state law. At least two years of experience as a Nurse Practitioner, ideally in Family or Adult Medicine. Experience with Part B billing is beneficial. Able to work autonomously in an advanced practice capacity. Licenses and Certifications: Active Nurse Practitioner license in accordance with state law. Current Nurse Practitioner certification. Collaborative agreement with a supervising physician as required. Part B Provider certification is necessary. Federal and state DEA licenses as applicable. Preferred: ACHPN certification.
