Job Title: Registered Nurse - Case Management Location: Concord, CA Department/Unit: Case Management Salary Range: $50 to $60 per hour Shift: 40 hours/week, 7:00 AM to 3:30 PM Contract Duration: 13 Weeks No.of Job vacancies- 2 Job Description Elitecare Medical Staffing is seeking a dedicated and experienced Registered Nurse - Case Manager to join our dynamic team in Concord, CA. The ideal candidate will have recent experience in acute care hospital case management and a passion for delivering exceptional patient care. Key Responsibilities: Provide comprehensive case management services to patients in an acute care hospital setting. Float between Concord and Walnut Creek as needed, adhering to the local travel policy (50-mile radius). Collaborate with healthcare providers, patients, and their families to develop and implement effective care plans. Utilize the Epic Electronic Medical Record (EMR) system for documentation and communication. Ensure compliance with all regulatory requirements and organizational policies. Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as necessary. Qualifications: Licensure: Current California Registered Nurse (RN) license required at the time of submission. Experience: Minimum of 2 years of recent acute care hospital case management experience is mandatory. Previous travel nursing experience is preferred. Skills: Proficient in using electronic medical records, preferably Epic. Strong communication and interpersonal skills are essential. Ability to work collaboratively in a fast-paced environment while managing multiple priorities.
